Extra study nobody asked him to do.
Dr. Romans earned his dental degree at A.T. Still University, and while he was there he also earned a certificate in public health. Nothing required it. It is a course of study about how care reaches whole communities rather than one mouth at a time, and he took it on top of a schedule that was already full.
Before that came a B.S. in Biology at the University of Kansas. So the science was never the interesting part of the decision. The interesting part was who the science was for.

Then he went looking for the hardest cases.
After orthodontic specialty training at Saint Louis University, where his graduate research examined Invisalign treatment outcomes, he could have gone straight into practice. Instead he took a craniofacial fellowship at St. Louis Children's Hospital, working with the Washington University team on children born with cleft lip and palate. Those are among the highest-stakes cases in the field, the ones where the bite, the airway and the growing face all have to be planned together over years, not months.
He kept volunteering the whole way through: Give Kids A Smile, Missions of Mercy, Dentures for Veterans, and the Arizona Humane Society. At every fork in the road he picked the version of dentistry that was about people rather than teeth.
